Ramp Fitness LLC Liability Waiver 

This Agreement is between {name} (‘Participant’) and Ramp Fitness LLC including its owners, trainers, employees, contractors, volunteers, and agents (‘Ramp Fitness’).

1. Acknowledgment of Risks

I understand that gym activities—including, but not limited to, the use of free weights, machines, cardiovascular equipment, group exercise classes, personal training sessions, bike sessions, and related programs (‘Activites’)—carry inherent risks. These risks include, but are not limited to, muscle strains, joint injuries, broken bones, injuries resulting from slipping and falling on gym premises, heart attacks, strokes, or other cardiovascular complications, exposure to and contraction of infectious diseases or viruses, injuries resulting from equipment malfunction or improper use of such equipment, and serious injury or death (‘Risks’).

2. Voluntary Participation & Health

I represent that I am voluntarily participating in the Activities, and I am physically fit to do so. I understand it is my responsibility to consult a physician before engaging in the Activities.

3. Assumption of Risk

I knowingly and freely assume the Risks associated with the Activities, as defined in paragraph one, including those arising from the ordinary negligence of Ramp Fitness.

4. Release of Liability

I release Ramp Fitness from any and all liability, claims, demands, or causes of action arising out of ordinary negligence. This includes any liability, claim, demand, or cause of action for personal injury, bodily injury, property damage, or wrongful death related to my participation in the Activities, except for claims arising from the gross negligence or willful misconduct of Ramp Fitness.

5. Indemnification

I agree to indemnify and hold harmless Ramp Fitness against any claims, liabilities, damages, or expenses (including attorneys’ fees) that may result from my participation in the Activities.

6. Medical Treatment

I consent to receive first aid and/or medical treatment deemed advisable in the event of an injury, accident, or illness during participation. I release and hold harmless Ramp Fitness for any costs or liability related to such treatment.

7. Severability

If any provision of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect.

8. Governing Law & Venue

This Agreement shall be governed by and construed in accordance with the laws of the State of Connecticut, and any disputes shall be resolved exclusively in the state or federal courts located in Hartford, CT.

9. Arbitration Agreement

I agree that any dispute, claim, or controversy arising out of or related to this Agreement or my participation in the Activities shall be resolved exclusively by binding arbitration administered by the American Arbitration Association in accordance with its Consumer Arbitration Rules. The arbitration shall take place in West Hartford, CT before a single arbitrator. I understand that by agreeing to arbitration, I am waiving my right to a jury trial or to participate in a class action.

10. Acknowledgment of Understanding

By signing below, I acknowledge that I have read this Agreement, fully understand its terms, and sign it freely and voluntarily. I understand that I am giving up substantial legal rights, including the right to sue.

Ramp Fitness LLC Acknowledgment and Consent for Appropriate Physical Assistance During Training

I understand that my training program may involve physical guidance or assistance from my trainer when necessary to support my safety, participation, learning, or proper exercise technique.

Examples of appropriate physical assistance may include:

Assisting with balance or stability.
Providing support during transfers or movement, when applicable.
Guiding body positioning or posture.
Assisting with the use of equipment.
Helping reduce the risk of injury during exercises or activities.

Before providing physical assistance, my trainer will explain the purpose of the assistance whenever practical and will respect my preferences regarding touch. I understand that I may ask questions, request a different method of instruction, or decline physical assistance at any time, unless immediate action is necessary to prevent injury or respond to an emergency.

I agree to communicate any concerns, physical limitations, pain, discomfort, or changes in my health that may affect my participation.

I understand that all physical assistance will be provided professionally, respectfully, and only to the extent necessary to support my training goals, safety, and independence.

Ramp Fitness LLC Media Consent Form

I grant permission to Ramp Fitness LLC to photograph, film, and record me while participating in gym activities, classes, events, or programs.

I understand that these photos, videos, and recordings may be used for promotional, marketing, educational, or informational purposes, including but not limited to:

• Social media platforms
• The gym's website
• Printed marketing materials
• Advertisements
• Newsletters
• Digital displays within the gym

I understand that I will not receive compensation for the use of these images or recordings.

I acknowledge that Ramp Fitness LLC may edit, crop, or modify the media as needed for promotional purposes.

This consent remains in effect until I provide written notice requesting that future photos or videos of me not be used. I understand that media already published before such notice may not be able to be withdrawn.
